Dr. Susan Mogan is a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP) with a Doctor of Nursing Practice (DNP). She holds additional board certifications in adult medicine, hospice and palliative care, and pain management.

Dr. Mogan brings more than 30 years of clinical experience to Cognifica Health, specializing in psychiatric evaluation, medication management, and integrated mental health care for patients with neurological and cognitive conditions.

Her educational background includes a BA in Economics from Stony Brook University, a BS in Nursing from Pace University, and an MS in Nursing from Western Connecticut State University, followed by her doctoral studies.

Dr. Mogan works closely with the Cognifica Health team to deliver comprehensive, coordinated care for patients dealing with depression, anxiety, PTSD, and mood disorders alongside cognitive and neurological challenges.

Jerry Raustiala is a highly experienced psychiatric nurse and healthcare specialist with over 28 years of expertise spanning home health, post-acute care, psychiatric nursing, and clinical education. He is known for his ability to seamlessly integrate clinical insight with patient-centered care, supporting individuals with complex medical and mental health needs.

Throughout his career, Jerry has developed strong partnerships with hospitals, physician practices, and community providers, helping patients access the right level of care at the right time. His work includes conducting comprehensive medical, psychiatric, and cognitive assessments, as well as guiding patients and families through chronic illness management, mental health challenges, and safety planning.

Jerry brings a specialized background in psychiatric and neurocognitive care, with advanced training in crisis intervention, motivational interviewing, and trauma-informed approaches. In addition to his clinical work, he has served as a nurse educator, providing disease-specific education and treatment support for conditions such as multiple sclerosis, asthma, and other complex disorders.

His experience also includes leadership roles in admissions, care coordination, and healthcare outreach, where he has helped strengthen referral networks and improve access to care across diverse populations.

Jerry holds a Master of Science in Nursing in Nursing Education and is currently completing an additional MSN in Psychiatric-Mental Health Nursing. He is a board-certified Psychiatric-Mental Health Nurse (PMH-BC) and a Multiple Sclerosis Certified Nurse (MSCN).

Education: Master of Social Work, Stony Brook University

Carol Gray, LCSW, is a dedicated social worker specializing in supporting individuals recovering from concussion injuries, life transitions, anxiety, depression, terminal illness, hospice care, and bereavement counseling. She is deeply committed to helping individuals navigate challenges in their lives with compassion, understanding, and respect.

Carol provides comprehensive emotional and social support to individuals. Her work includes developing individualized care plans, facilitating meaningful conversations, and guiding clients through complex medical, emotional, and life transitions with clarity and support.

She brings a thoughtful, integrative approach to her practice, drawing from evidence based modalities such as Cognitive Behavioral Therapy (CBT), interpersonal therapy, mindfulness practices, Dialectical Behavior Therapy (DBT), motivational interviewing, and psychoanalytic techniques. Carol creates a safe and supportive space where clients can process emotions, explore complex thoughts, navigate grief, and build effective coping strategies.

Carol holds a Master of Social Work from Stony Brook University. She is a Licensed Clinical Social Worker in New York State and a member of the National Association of Social Workers (NASW).

Director of Operations

Juliet Mott has worked alongside Dr. Abrahams since 2004, building a deep expertise in neurological practice administration, patient coordination, and program development.

In her role as Director of Operations and Program Coordinator, Juliet oversees the day-to-day clinical operations of all Cognifica Health locations. She ensures every patient receives a seamless, responsive experience from first contact through ongoing care.

Juliet is the connective tissue of the Cognifica Health team — the person patients and referring physicians work with directly to navigate scheduling, insurance verification, and care coordination across the practice.
